We aim to keep the referral process simple, clear and responsive.

How to Refer
Referrals can be submitted via email to:

Please include the following information:

• Participant name

• Contact details

• Funding type (plan-managed or self-managed, if applicable)

• Support coordinator details (if applicable)

• Relevant goals (if available)

• A brief overview of current challenges

If you are unsure whether the service is suitable, you are welcome to email for an initial discussion.

What Happens Next

1. We review the referral and confirm suitability.

2. An initial discovery call is arranged.

3. A service agreement is provided (if proceeding).

4. Sessions are scheduled collaboratively.
